Landmarks: Identify the bony prominence of the shoulder (acromion process) and inject 1–2 inches below.
Vastus Lateralis Muscle (Thigh)
Location: Middle third of the thigh, between the hip and knee.
Best For: Larger volumes (up to 3 mL), suitable for adults and children.
Landmarks: Divide the thigh into thirds and inject into the outer middle third.
Ventrogluteal Muscle (Hip)
Location: Outer hip area.
Best For: Volumes up to 3 mL; preferred for adults due to its safety and ease of access.
Landmarks: Place your palm on the hip bone (greater trochanter), thumb pointing toward the groin. Use the "V" formed by your index and middle fingers as a guide for injection.
Dorsogluteal Muscle (Buttocks)
Location: Upper outer quadrant of the buttocks.
Best For: Larger volumes (up to 3 mL); less common due to the risk of hitting the sciatic nerve.
Landmarks: Divide the buttock into four quadrants and inject into the upper outer quadrant.
Needle Selection
The size and length of the needle are crucial for a safe and effective injection. These depend on the medication, injection site, and patient’s age or body composition.
Needle Length:
Adults: 1 to 1.5 inches
Children: 5/8 to 1 inch
Infants: 5/8 inch
Needle Gauge:
Standard Medications: 22 to 25 gauge
Oil-Based or Viscous Medications: 20 to 21 gauge
Injection Technique
Using the correct technique ensures that the medication is administered safely and comfortably.
Prepare the Equipment:
Wash hands and wear gloves.
Assemble the syringe, needle, alcohol wipes, and medication vial.
Confirm the medication’s expiration date and dosage.
Locate and Clean the Site:

Clean the area with an alcohol wipe in a circular motion and let it air dry.
Draw Up the Medication:
Attach the needle securely to the syringe.
Draw the medication, ensuring no air bubbles remain.
Administer the Injection:
Stretch the skin taut (or pinch the muscle for thin patients).
Insert the needle at a 90-degree angle swiftly and steadily.
Aspirate (pull back slightly on the plunger) to check for blood. If no blood is present, proceed.
Inject the medication slowly and steadily.
Withdraw the Needle and Dispose Safely:
Remove the needle at the same angle of insertion.
Immediately place the used needle and syringe in a sharps container.
Apply gentle pressure to the site with gauze or a cotton ball.
